Drive.
Our driveway enters from Brown Park Drive, but our
signage is on
the southeast corner of the intersection in front of the
office. We are
adjacent to the Hilliard Post Office.

After Hours:
For routine matters, such as refill requests or
next-day appointment cancellations, messages may be
recorded on our office voice mail system by calling
(614) 876-7330. Our staff will handle your message
on our next business day.
If you believe you have a medical emergency,
call 911
or proceed to the nearest emergency room. If you
have
a serious, but nonemergent, problem after usual office
hours, one of our physicians is available by phone at

Additional Information For Patients:
New
patients are always welcome, and we appreciate referrals.
Here are a few things you
should know to make your office visits run smoothly.
● At the time of your
first appointment, we ask that you provide us with a photo ID, your
insurance card, and any records you may have about your medical
condition(s). You are encouraged to ask your previous physician or
hospital to forward pertinent medical records to us prior to your first
visit.
● At each appointment,
please update our records with any changes of name, employer, address,
phone number, insurance, or marital status. Also, we will be
accepting your required copayment (if any) at the time of your arrival.
Please have your current insurance card with you at each appointment.
● We ask that all minors
arrive with at least one parent or legal guardian for the initial office
visit, and preferably for each office visit thereafter. Please bring a
complete set of immunization records for all children and adolescents.
If a minor needs to be brought to an appointment by someone other than a
parent or legal guardian, a written note of permission from the
parent/guardian is required. If expressly stated in the note, we
can retain this
permission slip for future use until rescinded.
● When bringing minor
children of divorced parents to the doctor, please be sure to provide us
with any pertinent legal documents. Usually, these are in regard
to custody, authority to consent to and/or approve medical treatment,
and orders of financial responsibility. There may be additional
decrees that are unique to your situation. Foster parents and
other guardians may also have legal documentation (especially regarding
custody) that we will need to have on file.
● New patients are asked
to complete the necessary office paperwork prior to seeing the doctor.
Please arrive at least 15 minutes before your scheduled
appointment to allow adequate time to finish this important step in
registration. If you wish, you may download the appropriate forms
from this website (see Useful Forms), and bring them with you to
your first visit. Patients who have not seen one of our doctors in
the last three years may be asked to complete updated paperwork, so
please plan your arrival time accordingly.
